VA Hospital Requirements to Work: (4/8/2013)

Below are the requirements of all employees working at the VA Hospitals. If you have any questions, contact your training program office to work with the Administrative Officer of your VA Service.

Application for Health Professions Trainees: 10-2850D Application for Health Professions
Declaration for Federal Employment: OF 306
Appointment Affidavits: SF 61
DO NOT SIGN: Until you are instructed to sign while in the presence of a VA Official
2) Medical Resident Fingerprint Record: If possible, trainees should get fingerprinted at their nearest VA facility using the codes on the attached form prior to arrival for orientation. You will need two forms of government-issued identification – at least one of them including a photo (such as a passport or Driver’s License). Fingerprint Form – VA Housestaff
3) VHA Mandatory Training for Trainees (MTT). Trainees must self-enroll in VA’s Talent Management System (TMS) and complete Mandatory Training for Trainees (MTT) prior to orientation using the linked instructions.
