Documentation of the following types of evaluation and results are needed prior to your site visit. Contact the GME office (352-265-0152) if you’d like examples of any of the forms listed below.
Required Evaluations of Resident/Fellow
- Six-Month/Semi-Annual Evaluations
- Semi-Annual/Mid-Year Evaluation of Resident/Fellow (for CCC meetings) – statement regarding whether or not “…. progressing as expected.”
- Summative/End of Year Evaluation of Resident/Fellow (for CCC meetings) – must have statement regarding whether or not “….demonstrated readiness to progress to the next year of the program.”
- Final/Program Exit Evaluation of Resident/Fellow (for CCC meetings) upon exit OR upon graduation of the program – must have statement regarding has or has not “…demonstrated the knowledge, skills, and behaviors necessary to enter autonomous practice.”
- 360/Multisource Evaluations
- Professional Staff Evaluation of Resident/Fellow (at least twice a year before CCC Meeting)
- Patient Evaluation of Resident/Fellow (at least twice a year before CCC Meeting)
- Peer Evaluation of Resident/Fellow (at least twice a year before CCC Meeting)
- Self-Evaluation (at least twice a year before CCC Meeting)
- Faculty Evaluation of Resident/Fellow by end of each Rotation/Experience Type at end of each rotation;
- e.g. if 2 week long rotations, then documented evaluation at the end of the 2 weeks;
- e.g. if rotations are GREATER than 3 months in length, ACGME requires documented evaluation every 3 months; UF GME office recommends at least every 2 months for increased data points for CCC consideration)
- e.g. Longitudinal experiences such as continuity clinics, if applicable (ACGME requires every 3 months per ACGME requirement AND at completion)
Completed evaluations of all of the above listed required evaluations must be presented for documentation for ACGME site visits (including documentation for those transferring into or transferring out of a training program).
Required Evaluations of Faculty
- Resident/Fellow Evaluation of Faculty (at end of rotations)
Completed evaluations of the above listed required evaluations must be presented for documentation for ACGME site visits.
Required Evaluations of Program for Annual Program Evaluation
- Resident/Fellow Evaluation of Program (once a year – must be written; recommend including SWOT questions)
- Faculty Evaluation of Program (once a year – must be written; recommend including SWOT questions)
- Evaluation of Rotation (at end of rotation or required longitudinal experience)
- Graduate Evaluation of Program (ACGME suggests 1 & 5 year intervals)
- Strengths-Weaknesses-Opportunities-Threats [SWOT] (required for use by the Program Evaluation Committee)
