Types and Timing of GME Training Program Evaluation Forms

Documentation of the following types of evaluation and results are needed prior to your site visit.  Contact the GME office (352-265-0152) if you’d like examples of any of the forms listed below.

Required Evaluations of Resident/Fellow

  • Semi-Annual Evaluation of Resident/Fellow (twice a year)

  • Summative/Final Evaluation of Resident/Fello (once upon exit or graduation of the program)

  • 360 Staff Evaluation of Resident/Fellow (twice a year)

  • Patient Evaluation of Resident/Fellow (twice a year) Faculty Evaluation of Resident/Fellow by each Rotation/Experience Type — including continuity clinics, research, etc. if applicable (twice a year)

  • Peer Evaluation of Resident/Fellow (twice a year)

  • Self-Evaluation (twice a year)

Required Evaluations of Faculty

  • Resident/Fellow Evaluation of Faculty (at end of rotations)

Required Evaluations of Program for Annual Program Evaluation

  • Resident/Fellow Evaluation of Program (once a year)

  • Faculty Evaluation of Program (once a year)

  • Graduate Evaluation of Program (typically sent at 1 & 5 year intervals)

  • SWOT (required for self-study)
